Found in thin wood areas.

Physical Description
I came across this plant while walking through the woods. They are in 2 places, both of which are lightly wooded areas and fairly moist. Soil is rocky. The flowers are on top of a single stem with leaves are adjacent from one another as they go up. The leaves get smaller and there seems to be about three sets up the plant. Can anyone help to identify this plant? If you could please include common name? I am very new at this. :) Thanks!
